The Better Care Fund is a national plan to create integrated health and social care.

It creates a single pooled budget - worth 3.8bn nationally - which is made up of existing funds from both NHS CCGs (Clinical Commissioning Groups) and Adult Social Care (Social Services).

The Better Care Fund, which was announced by the Government in the June 2013 spending round, aims to ensure a transformation in integrated health and social care.

It is one of the most ambitious programmes across the NHS and Local Government and aims to enable the NHS and local government to work more closely together.

In Surrey Heath, the CCG is working closely with other health and social care colleagues to create integrated teams of health and social care professionals.  These teams went live in April 2015.
